To answer your question: Nic was telling the story of how during the BL shooting he was snorting sacharine from a vial to simulate that it was coke and he was getting high. But his performance was so convincing the the director thought that what Nic kept in that vial was the real thing, that it was coke. So, Herzog asked Nic accusatively: "What IS in that vial?" And Nic, who was focused on his performance and didn´t want to be taken away from his simulation, answered in the same aggressive tone: "It´s COKE!" That´s what he´s saying in the pic, and his lips are rounded cause he´s uttering "coke".
