Basically, I think it is malnutrition. He must have been on a very strict diet to get so super thin for the Ghost Rider role. The limiting of essential nutrients in the diet not only affects the physical but also the psychological side. Depression is hard to contain in the bordering anorexic. Nic needs to get back to a proper diet for a man of his height (till he starts on the Ghost Rider Sequel of course - and even then supplement his limited diet with vitamin/mineral pills to keep the demons at bay!)

He look so old, depress and skinny, maybe health problems ? Or family troubles ...divorce in a close future ? He needs to fix his personal things up !!! This is sad!
He looks so careworn, doesn't he? What problem could be so disquieting? Or may be he's just thinking of his new films...some good ideas... or something like that. Anyhow, he's got a really desperative face...
